Its simple web application which connect doctors, Patients and providers ( Organizations ) . Doctor connect with patients to see his/her health status through patients can fill up Survey(eprom) assign to him/her. based on result doctors can give suggestion , medicines and daily diet plan . Patients can fill up Survey which assigned by doctors through providers . Providers ( Organization ) : its like person whom manage event like Blood Donation Camp , Eyes Checkup , Dental checkup etc .. So . basically patients connect with doctors though providers and we have admin panel which is access by system users only . through admin panel set various Surveys(eproms) from SurveyMonkey APIs. Using SuveyMonkey APIs we can get perfect results to show on dashboard with data visualize tools.
